If you are travelling abroad, you may require vaccinations. Firstly, you need to complete the attached form or pop into surgery to collect one. The nurse will then perform a risk assessment, from the details you have provided on the form. (Please provide as many details as you can, including the towns to be visited in addition to the country). Please then contact the practice, leaving 2 weeks after completing the form, to confirm whether an appointment is required.

It is important to make this initial appointment as early as possible - at least 8 weeks before you travel - as a second appointment will be required with the practice nurse to actually receive the vaccinations. These vaccines have to be ordered as they are not a stock vaccine. Your second appointment needs to be at least 2 weeks before you travel to allow the vaccines to work. Please be aware that on occasions, there can be manufacturing problems which may mean that a recommended vaccination cannot be given in time for your holiday. In this case, lifestyle advice will be offered by the practice nurse to reduce the risk of you contracting disease.
The practice is unable to guarantee that an appointment will be provided in time for your holiday. However we will try our very best to provide an appointment in the required timescale.
Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS; One example is yellow fever. Please enquire with medical reception staff for the cost of vaccinations and travel certificates from the surgery. We can also provide the telephone number for the Private Travel Clinic.
